- Shift: Monday–Friday, 8:00 AM–5:00 PM (weekend on-call required)
- Hours per Week: 40 (8-hour shifts)
- Weekly Estimated Pay: $1,760 – $1,850
- Mileage Reimbursement: $0.42 per mile
Required Qualifications
- Active Registered Nurse (RN) license in Colorado (required)
- Minimum 1 year of nursing experience; home health or hospice nursing experience strongly preferred
- Proficiency with home health clinical documentation systems (HCHB and OASIS) and strong clinical documentation skills
- Ability to meet productivity expectations (minimum of 30 visits/productivity units)
- Strong assessment, care planning, organization, and communication skills
- Valid driver’s license and reliable transportation; ability to travel within Mesa and Garfield counties
Key Responsibilities
- Provide skilled nursing care in patient homes and community settings, including assessment, care planning, implementation, and evaluation
- Deliver clinical care for well, chronic, and acutely ill patients as a Home Health RN / Hospice Nurse
- Coordinate and collaborate with physicians, social workers, therapists, discharge planners, and other members of the interdisciplinary team
- Supervise and train staff on clinical best practices, safety, and regulatory requirements
- Administer and document medications accurately; ensure adherence to medication policies and patient safety standards
- Recognize and report changes in patient condition to physicians; implement and transcribe physician orders as appropriate
- Maintain timely, accurate documentation (daily, monthly, quarterly, annual) using HCHB/OASIS to support quality outcomes and regulatory compliance
- Schedule and follow up on patient appointments, coordinate transitions of care, and maintain communication with inpatient nursing and discharge coordinators
- Promote health maintenance, safety, and disease prevention for all patients served
