Physical Therapist (PT) Job Requirements
- Minimum 1 year of experience as a licensed Physical Therapist (PT)
- At least 6 months of experience in a Home Health setting as a PT (home health PT jobs preferred)
- Proficiency with OASIS documentation and HCHB (Homecare Homebase)
- Current Physical Therapist license in the state of Arkansas (AR)
- Current CPR certification
- Valid driver’s license, vehicle insurance, and access to reliable transportation or public transit
Home Health Physical Therapist Responsibilities
- Conduct comprehensive home health physical therapy evaluations and skilled treatments in patients' homes
- Develop and implement individualized care plans focused on functional goals, fall prevention, and safe discharge outcomes
- Accurately document visits, progress notes, and OASIS assessments using HCHB
- Collaborate with case managers, nurses, and interdisciplinary team members to optimize patient outcomes
- Educate patients and caregivers on therapy techniques, home safety, adaptive equipment, and caregiver training
- Maintain timely, compliant documentation of patient progress and outcome measures
