Job Requirements
- Minimum 2 years of experience as a Home Health RN or in-home nursing
- Experience with OASIS assessments and HCHB electronic documentation required
- Current RN licensure in the state of Florida (compact license accepted if applicable)
- Current CPR certification (BLS)
- Valid driver's license, proof of vehicle insurance, and reliable transportation or access to public transit
Responsibilities
- Deliver skilled nursing care to patients in their homes following individualized care plans
- Conduct comprehensive OASIS assessments and document care accurately in HCHB
- Coordinate and collaborate with interdisciplinary teams to support positive patient outcomes
- Provide patient and family education on disease management, medication administration, and health maintenance
- Maintain accurate clinical records and ensure compliance with home health regulatory standards
