Travel Hospice Homecare Registered Nurse (RN) – Bend, Oregon

- On-Call/Weekend Requirement: Once per week
- Coverage Area: La Pine, Madras, Sisters, and Prineville
- Productivity Expectation: 28
Job Requirements
- Active Oregon RN license
- Current BLS certification required
- Associate’s degree in Nursing or higher
- At least 1 year of nursing experience required
- Hospice nursing experience required
- Strong communication, organization, and time-management skills
- Experience with HCHB is a plus
- Business casual dress code
Responsibilities
- Provide hospice case management and skilled nursing support in the homecare setting
- Complete patient assessments, care coordination, and ongoing plan-of-care updates
- Deliver compassionate, high-quality care to patients and families
- Coordinate care across the service area and support continuity of services
- Document visits accurately and in a timely manner
- Manage weekly on-call and weekend coverage as scheduled
- Support productivity goals while maintaining excellent patient care
