Overview
We are seeking an experienced Registered Nurse for a travel home health assignment providing adult and geriatric care in the Portland, Oregon area. You’ll manage a steady patient caseload, work with modern charting tools, and deliver high-quality patient care in a setting that values autonomy, organization, and patient-centered service.

- Schedule: 5 x 8-hour day shifts
- Shift Time: 8:00 AM - 4:30 PM
- Hours per Week: 40
- Assignment Length: 13 weeks

- Patient Population: Adults and geriatrics

- Visits per Day: Up to 5-6 patients

Job Requirements
- Active Oregon RN license required; compact license in hand at submission if applicable
- Minimum of 1 year of skilled home health nursing experience
- BLS certification required
- OASIS experience required
- Experience with electronic health records and charting systems required
- Reliable transportation required
- Strong skills in admission assessments, start of care, recertification, resumption of care, and routine home health visits
- Experience with wound care, medication management, IV therapy, G-tube care, and catheter care preferred
- Ability to work independently, stay organized, and manage productivity standards in a fast-paced healthcare setting
Responsibilities
- Provide skilled nursing care to adult and geriatric patients in their homes
- Complete comprehensive assessments, start-of-care visits, recertifications, and discharge planning
- Administer medications and infusions, monitor patient response, and update care plans as needed
- Support wound management, infection control, pain assessment, and education for patients and caregivers
- Document accurately in electronic systems, including OASIS and remote home health charting tools
- Coordinate with the interdisciplinary care team to promote safe, timely, patient-centered outcomes
- Maintain productivity standards while delivering compassionate, high-quality home health care
