Job Details

- Setting: Home Health / Home Care Nursing
- Location: Wallingford, CT 06492 and surrounding areas
- Schedule: Monday through Friday, approximately 9:00 AM to 5:00 PM
- Hours: 40 hours per week
- Shift Duration: 8-hour shifts
- Shift Frequency: 5 shifts per week
- Contract Length: 13 weeks
- Weekly Estimated Pay: $2,120 - $2,230
- On Call/Weekend Requirement: None
- Mileage: Reimbursed between visits
Job Requirements
- Associate's degree in nursing required
- Active Registered Nurse license in the state of Connecticut
- Professional certification required
Responsibilities
- Complete skilled nursing visits in the home setting
- Document assessments, interventions, and patient progress in the EMR
- Participate in case conferences and interdisciplinary communication
- Provide safe, compassionate, and patient-centered care
- Support care plans and help promote positive patient outcomes
- Travel within the assigned coverage area to visit patients in their homes
