Travel Hospice Registered Nurse (RN) – Home Hospice RN Case Manager | Durham, NC
Provide compassionate, comfort-focused end-of-life care as a Travel Hospice Registered Nurse (RN) in Durham, North Carolina. This hospice nursing travel assignment covers a field territory including Cary, Apex, Morrisville, and Raleigh. You’ll manage a daily visit schedule, partner with an interdisciplinary hospice team, and support patients and families through expert symptom management and caregiver education.

Hospice RN Travel Job Details
- Location: Durham, NC 27704 (field territory: Cary, Apex, Morrisville, Raleigh)
- Start Date: 03/23/2026
- End Date: 06/22/2026
- Duration: 13-week travel contract
- Shift: Day shift
- Shift Duration: 8-hour days (8:00 AM – 5:00 PM)
- Hours per Week: 40
- Call/Weekends: No call; no weekends
- Expected Workload: 4–5 patient visits per day (typically 5)
- Patient Population: Adults & Geriatrics
- Charting/EMR: Epic (experience required)
- Orientation: 48 hours (bi-weekly orientation schedule)
- Scrubs: Any color
Hospice Registered Nurse (RN) Requirements
- Experience: 2 years of RN experience required, including at least 1 year of recent hospice field/home hospice experience (required)
- License: Active North Carolina RN license or Compact (Multistate) RN license (must be in hand)
- Education: ADN required
- Certification: BLS
- EMR/Charting: Epic experience required
- Clinical Skills: Home health/hospice care, cultural awareness, identifying spiritual needs, perideath family support, patient and caregiver education
- Vaccination: Flu vaccine required (no exemptions/declinations)
- References: One supervisory reference within the last year
- Time Off: Request time off at submission (typically 5–7 days maximum)
Key Responsibilities – Home Hospice RN Case Manager
- Provide routine and PRN home hospice visits, including comprehensive assessments, vital signs, and comfort-focused symptom management
- Complete hospice admissions, recertification visits, and time-of-death visits with professionalism, empathy, and clear communication
- Perform medication reconciliation, coordinate medication refills, and support individualized hospice plans of care
- Document accurately and on time in Epic, ensuring compliance with hospice documentation standards and patient care requirements
- Collaborate with the interdisciplinary team (including social services and interpretation services) to support patients, caregivers, and families
- Utilize and support patient care equipment as needed, including PCA pumps, feeding pumps, Macy catheter, Pleurx/Aspira drains, ports, PICCs/CVLs, Trilogy, and CPAP
