- Work Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- Hours per Week: 40
- On-Call Requirement: Rotating weeknight and weekend coverage
- Productivity Expectation: 30 points
Job Requirements
- Active Registered Nurse (RN) license
- Minimum of 1 year of nursing experience; home health experience preferred
- Associate degree in Nursing or higher from an accredited program
- Current CPR certification
- Strong independent assessment and clinical decision-making skills
- Clean driving record and ability to travel to patient homes
- Experience with OASIS documentation preferred
- Comfort using EMR systems, including home health documentation platforms
- Background in wound care and IV medication administration preferred
Responsibilities
- Provide skilled nursing care directly in patients' homes
- Conduct thorough patient assessments and ongoing monitoring
- Administer medications and manage treatment plans as ordered
- Perform wound care and other skilled nursing procedures
- Complete accurate regulatory documentation, including OASIS assessments
- Educate patients and families on care plans, medications, and symptom management
- Coordinate care with interdisciplinary team members to support continuity of care
- Help reduce avoidable hospital readmissions through proactive clinical support
