- Shift Time: 8:00 AM - 4:30 PM
- Weekend Rotation: Approximately once every 6 weeks
- Weekly Estimated Pay: $2,500 - $2,630
Job Requirements
- Active RN license in the appropriate state
- At least 1 year of certified home care or home health nursing experience
- At least 1 year of case management experience
- Ability to travel to assigned patient cases throughout the Newton and North Region service area
- Strong electronic medical documentation and charting skills
- Experience working collaboratively with interdisciplinary care teams
Responsibilities
- Assess patient conditions and identify ongoing care needs in the home setting
- Implement individualized care plans to support safe, effective recovery and patient outcomes
- Provide patient and family education, guidance, and support
- Complete admission processes and case management responsibilities
- Document care thoroughly, accurately, and in a timely manner
- Communicate clearly with the interdisciplinary healthcare team
- Deliver compassionate, patient-centered home health care to patients in their homes
