Provide compassionate hospice nursing care where patients feel most comfortable—at home and in community settings. This travel Registered Nurse (RN) opportunity supports hospice patients across the Milwaukee metro area with a dedicated weekend program focused on end-of-life and symptom-management visits (no assigned caseload/case management). You’ll work independently in the field while staying connected to a supportive interdisciplinary hospice team.

Home Health Hospice RN Job Details

• Job Title: Home Health / Hospice Registered Nurse (RN) – Field Visits
• Employment Type: Travel Contract
• Start Date: 03/23/2026
• End Date: 06/20/2026
• Contract Duration: 13 weeks
• Shift: 12-hour days
• Schedule: Weekend program – Saturday & Sunday, 8:00 AM–8:00 PM
• Average Hours/Week: 24 (weekend program)
• Setting: Community/field hospice visits (homes, assisted living, group homes, nursing homes, and other community-based locations; occasional hospital-based hospice patient)
• Documentation/EMR: Epic (Hyperspace/ClinDoc or remote client, role dependent)
• Orientation: Orientation provided; includes online onboarding, Epic EMR training, and field precepting as needed

Registered Nurse (RN) Requirements

• RN Experience: 1+ year as a Registered Nurse (hospice RN / home health RN experience strongly preferred)
• Licensure: Active Wisconsin RN license (or ability to obtain prior to start)
• Certification: BLS (AHA)
• Clinical Skills: End-of-life care, pain and symptom management, and independent field-based clinical decision-making
• Technical Skills: Strong EMR documentation skills (Epic preferred)
• Other Qualifications: Flexible, reliable, and confident working autonomously while knowing when to escalate concerns

Hospice RN Responsibilities (Home Health / Field)

• Provide compassionate hospice nursing care in the field, including daily end-of-life visits and PRN visits for symptom management
• Perform focused assessments and implement care interventions to support comfort, dignity, and quality of life
• Deliver skilled nursing care such as wound care/wound vac support, catheter insertion, tube feeding support, and triage
• Manage and maintain lines/drains as needed (e.g., PIV, midline, central line care; chest tube/PleurX drain support)
• Support medication-related symptom control, including safe opioid use and equianalgesic conversions per protocol
• Coordinate with the interdisciplinary hospice team and communicate changes in condition promptly
• Document visits accurately and timely in Epic EMR and follow established clinical workflows
• Educate patients and caregivers on comfort measures, symptom monitoring, and when to call for help
