Job Requirements
- Active Registered Nurse (RN) license
- Hospice nursing, post-acute care, or similar patient care experience preferred
- BLS certification required
- Strong communication, organization, and critical thinking skills
- Comfortable with weekday scheduling and on-call RN responsibilities
- Experience with MEDITECH electronic health record system is a plus
Responsibilities
- Provide compassionate hospice nursing care to patients and support to families
- Assess patient conditions and coordinate individualized care plans
- Administer medications and treatments in accordance with hospice protocols
- Document care accurately and timely in the electronic health record
- Collaborate with the interdisciplinary care team to promote comfort and quality of life
- Respond to patient needs during scheduled shifts and on-call coverage
- Educate patients and families on care needs, symptom management, and support services
