Job Requirements / Qualifications

  • Minimum 12 months of Home Health Occupational Therapy experience within the past 3 years (home health OT experience required)
  • Active Occupational Therapist (OT) license in Indiana or eligibility to obtain one prior to start (Indiana OT license)

  • Strong communication, time management, and independent clinical decision-making skills

Key Responsibilities – Travel Occupational Therapist

  • Provide skilled occupational therapy services during home health visits across the assigned territory
  • Conduct patient evaluations, develop individualized care plans, and implement evidence-based OT interventions focused on ADLs, adaptive equipment, and safety
  • Accurately and promptly document patient care activities in HCHB (Homecare Homebase)
  • Collaborate with interdisciplinary teams (nursing, PT, SLP, case management) to ensure optimal patient outcomes and care transitions
  • Educate patients and caregivers on adaptive techniques, fall prevention, home exercise programs, and community resources
