Physical Therapist – Home Health Contract in Brunswick, GA

Join our healthcare team as a Home Health Physical Therapist and provide skilled, individualized care throughout Brunswick, Georgia, and the Golden Isles. This contract physical therapy opportunity offers a consistent Monday–Friday schedule, 40 guaranteed hours per week, and collaboration with an interdisciplinary care team across southeast Georgia.

Physical Therapist Job Details

• Weekly Estimated Pay: $1,880–$2,020
• Healthcare Setting: Home health
• Location: Brunswick, Georgia, with coverage across Glynn, Charlton, Brantley, and Camden counties
• Schedule: Monday through Friday
• Shift Duration: 8 hours
• Hours: 40 guaranteed hours per week
• Shifts per Week: 5
• On-Call: None
• Start Date: October 12, 2026
• End Date: January 10, 2027
• Contract Duration: 13 weeks
• Travel Area: Patient visits throughout the assigned service counties

Home Health Physical Therapist Requirements

• Valid Physical Therapist license; compact licenses are accepted
• At least 1 year of professional Physical Therapist experience
• Required home health physical therapy experience
• Proficiency with HCHB documentation and workflow
• Ability to provide safe, independent patient care in homes and alternative care settings
• Strong communication, clinical judgment, organization, and time-management skills
• Ability to meet a productivity expectation of 30 points

Physical Therapist Responsibilities

• Evaluate patients in their homes or alternative care settings to identify functional limitations, mobility challenges, and treatment needs
• Develop, implement, and update individualized physical therapy treatment plans based on physician-approved plans of care
• Provide therapeutic exercises, mobility training, functional activities, and other clinically appropriate home health interventions
• Teach and motivate patients and caregivers to improve mobility, strength, safety, and activities of daily living
• Recommend and support the appropriate use of adaptive equipment and assistive devices
• Monitor patient progress and revise physical therapy interventions to promote measurable outcomes
• Provide clinical supervision and guidance to Physical Therapist Assistants and Home Health Aides as appropriate
• Maintain timely, accurate documentation in HCHB and communicate effectively with the interdisciplinary care team
