Home Health Physical Therapist – Travel Contract in Chesapeake, VA

Physical Therapist Job Details
- Weekly Estimated Pay: $2,310–$2,480
- Assignment Length: 13 weeks
- Start Date: October 19, 2026
- Schedule: Monday through Friday, 8:00 a.m. to 5:00 p.m.
- Shifts per Week: 5
- Shift Duration: 8-hour days
- Hours: 40 hours per week
- Work Setting: Home health
- Caseload: Approximately 6–7 weighted visits per day
- Weekend Coverage: Approximately every fourth or fifth weekend for Physical Therapy; weekend coverage may also be needed for orthopedic cases
Home Health Physical Therapist Requirements
- At least 1 year of Physical Therapist experience required
- Home health physical therapy experience preferred
- Active Virginia Physical Therapist license or compact privilege required at submission
- CPR certification required
- Experience with MatrixCare or Epic preferred, but not required
- Strong clinical judgment, communication, organization, and clinical documentation skills
- Ability to provide safe, independent patient care in home-based environments
Physical Therapist Responsibilities
- Evaluate patients and develop individualized, medically prescribed physical therapy treatment plans
- Provide therapeutic interventions to restore function, relieve pain, and prevent disability
- Educate patients and caregivers on treatment plans, home exercise programs, mobility, and safety
- Direct patients through selected activities designed to restore, reinforce, and enhance physical performance
- Monitor patient progress and modify treatment plans as clinically appropriate
- Participate in discharge planning and coordinate care with the interdisciplinary team
- Complete accurate and timely clinical documentation in accordance with applicable policies and standards
