- Assignment Length: 13 weeks
- Schedule: Day shift, 8-hour shifts, Monday through Friday
- Hours Per Week: 40
- Estimated Weekly Pay: $2,280 - $2,450
- Charting System: HomeCare HomeBase (HCHB)
- Travel Requirement: Must be willing to travel up to 75 miles from the assigned care center
- Work Type: Travel assignment
- Patient Focus: Adult home health visits, including start-of-care and follow-up visits
Job Requirements
- Minimum 1 year of recent adult home health experience required
- Physical Therapist license in hand required
- Current CPR/BLS certification required
- Recent experience using HomeCare HomeBase EMR required
- Reliable transportation, valid driver’s license, and auto insurance required
- Must be comfortable driving to patient homes daily and attending periodic office meetings
- Must be open to a flexible schedule and on-call rotations as needed
- Strong documentation skills and ability to complete charting within 24 hours
- Ability to manage a productive caseload and work independently in the field
Responsibilities
- Provide skilled physical therapy services in the home health setting
- Complete evaluations, reassessments, and follow-up visits for adult patients
- Develop and update individualized plans of care based on patient needs and progress
- Support mobility, function, safety, and recovery goals in the home environment
- Educate patients and caregivers on therapy exercises and home safety strategies
- Document all visits accurately and promptly in the EMR
- Coordinate with the interdisciplinary care team to support quality patient outcomes
- Travel throughout the assigned territory to meet patient needs
