Physical Therapist Job – Home Health Contract to Hire in Beckley, West Virginia

Job Requirements

  • Active Physical Therapist license in West Virginia; compact licenses accepted
  • At least 1 year of Physical Therapy experience preferred
  • Home health physical therapy experience strongly preferred
  • OASIS experience preferred
  • HCHB experience preferred
  • Ability to manage a caseload with a benchmark of 28 units/patients per week
  • Strong communication, organization, and documentation skills
  • Ability to work independently while collaborating with an interdisciplinary care team

Responsibilities

  • Perform physical therapy evaluations and treatments based on physician orders and referrals
  • Complete comprehensive home health assessments using appropriate clinical tools and procedures
  • Review patient records, including diagnoses, medical history, precautions, and contraindications
  • Develop measurable goals and individualized treatment plans to support maximum function and independence
  • Reassess patient progress and adjust treatment plans as needed
  • Communicate patient progress and needs with physicians, care teams, patients, and families as appropriate
  • Recommend and assist with adaptive equipment to improve patient function
  • Maintain accurate evaluation, treatment, and billing documentation in accordance with applicable regulations
  • Supervise Physical Therapist Assistants and Physical Therapy Aides when applicable
  • Participate in staff development and continuing education to strengthen clinical skills
