Physical Therapist Job – Home Health Contract to Hire in Beckley, West Virginia

- Additional Details: No weekends or on-call
Job Requirements
- Active Physical Therapist license in West Virginia; compact licenses accepted
- At least 1 year of Physical Therapy experience preferred
- Home health physical therapy experience strongly preferred
- OASIS experience preferred
- HCHB experience preferred
- Ability to manage a caseload with a benchmark of 28 units/patients per week
- Strong communication, organization, and documentation skills
- Ability to work independently while collaborating with an interdisciplinary care team
Responsibilities
- Perform physical therapy evaluations and treatments based on physician orders and referrals
- Complete comprehensive home health assessments using appropriate clinical tools and procedures
- Review patient records, including diagnoses, medical history, precautions, and contraindications
- Develop measurable goals and individualized treatment plans to support maximum function and independence
- Reassess patient progress and adjust treatment plans as needed
- Communicate patient progress and needs with physicians, care teams, patients, and families as appropriate
- Recommend and assist with adaptive equipment to improve patient function
- Maintain accurate evaluation, treatment, and billing documentation in accordance with applicable regulations
- Supervise Physical Therapist Assistants and Physical Therapy Aides when applicable
- Participate in staff development and continuing education to strengthen clinical skills
