Required Qualifications
- Active and valid California Physical Therapist (PT) license (or compact/reciprocal license where applicable)
- Minimum 1 year of recent home health or hospital-based physical therapy experience
- Proficiency with HCHB (Homecare Homebase) documentation system required
- Strong ability to work independently while managing a multi-site caseload
- Excellent communication, clinical reasoning, and organizational skills
Key Responsibilities
- Provide high-quality, evidence-based physical therapy evaluations and interventions in a hospital setting and during home visits as needed
- Perform thorough assessments, develop individualized care plans, and implement treatment plans to improve patient mobility and function
- Document all patient care and progress accurately and timely using HCHB
- Collaborate with interdisciplinary team members (nursing, case management, OT, SLP) to ensure coordinated patient care
- Manage an average caseload (approximately 6 points per day) and prioritize caseload based on clinical need
- Participate in discharge planning, patient education, and family training to support continuity of care
Preferred / Helpful Skills
- Experience with home health visit routing and travel logistics in rural/suburban regions
- Familiarity with High Desert community needs and local healthcare networks (Victorville, Apple Valley, Adelanto)
- Certifications such as Clinical Instructor (CI), NCS, or specialty training are a plus
