Job Details
- Weekly Estimated Pay: $2,260 – $2,430
- Location: Klamath Falls, OR 97601 (Home Health)
- Contract Duration: 13 weeks (June 1, 2026 – August 31, 2026)
- Guaranteed Hours: 40 hours per week
- Shift: 8- or 10-hour day shifts, Monday–Friday
- Mileage Compensation: $0.70 per mile for travel to patient homes
- Charting Systems: Netsmart (formerly Allscripts) and EPIC
- Job Type: Travel Physical Therapist / Home Health PT / Contract
Required Qualifications
- Active Oregon Physical Therapist (PT) license required at time of submission
- BLS (Basic Life Support) certification required
- Minimum 1 year of experience as a Physical Therapist; home health experience preferred
- Valid driver’s license and reliable transportation for patient visits
- Strong organizational, documentation, and communication skills
- Ability to work independently in patients’ homes and learn agency protocols quickly
Key Responsibilities
- Provide skilled physical therapy services in patients’ homes (average ~5 visits/day)
- Assess mobility, develop individualized treatment plans, and implement home-based rehab
- Document patient progress, care plans, and outcomes using Netsmart and EPIC
- Collaborate with interdisciplinary healthcare teams to coordinate comprehensive patient care
- Educate patients and caregivers on therapeutic exercises, fall prevention, and safety
- Maintain compliance with clinical, regulatory, and professional standards
