Key Responsibilities (Physical Therapist)

• Evaluate patients and complete PT assessments to develop individualized plans of care focused on mobility, strength, balance, and functional independence
• Provide skilled therapeutic interventions and progress patients toward measurable functional goals
• Coordinate care and communicate with the broader care team as needed
• Educate patients and caregivers on home exercise programs (HEP), fall prevention, safety strategies, and discharge recommendations
• Document evaluations, daily treatments, progress notes, and discharge summaries per facility policy and regulatory standards
• Support discharge planning, including equipment recommendations and continuity of care needs
