Job Requirements
- Minimum 1 year of skilled home health experience
- Current Physical Therapist license in Oregon; Washington license also required for coverage area
- Current BLS certification
- Experience with OASIS documentation strongly preferred
- Comfort working independently in a home health environment
- Reliable transportation and willingness to travel throughout the service area
- Strong skills in lifting and transfer techniques, patient education, and care coordination
- Experience with Epic charting preferred
Responsibilities
- Provide skilled physical therapy evaluations and treatments in the home setting
- Develop and adjust individualized plans of care based on patient needs and progress
- Support patients with mobility, strengthening, gait training, balance, and fall prevention
- Educate patients and families on home exercise programs, safety, and functional improvement
- Document care accurately using home health charting tools, including OASIS and Epic-based systems
- Coordinate with the interdisciplinary care team to promote high-quality outcomes
- Manage a productivity expectation of up to 6 visits per day
- Deliver compassionate care to patients with diagnoses such as dementia, Parkinson’s, ALS, MS, orthopedic post-op needs, and post-stroke recovery
