Your Impact

You’ll serve as a key clinical leader, guiding patients through their kidney care journey with a whole-person approach.

Core Responsibilities:

• Conduct Comprehensive Health Evaluations (CHEs) addressing medical, social, and emotional needs

• Manage patients with CKD and ESKD, focusing on slowing progression and improving outcomes

• Provide medication management, diagnostics, and care coordination

• Reduce hospitalizations through proactive, relationship-driven care

• Deliver care across dialysis clinics, nephrology practices, and telehealth platforms

• Collaborate closely with interdisciplinary teams to align care plans

Hybrid Work Model

• Telehealth (Remote): Conduct virtual visits, care planning, and follow-ups from your home office

• In-Person: Travel within the Sacramento/San Francisco region for clinic-based patient care

• Travel Expectation: ~1x/month to Berkeley, plus local travel as needed

What You Bring

• Passion for ESKD and complex chronic care populations

• Minimum 2 years of Nurse Practitioner experience

• Strong clinical judgment and comfort working autonomously

• Active NP license + DEA (or ability to obtain within 90 days)

• Current BLS/CPR certification

• Reliable transportation + ability to travel locally

• Tech-savvy with EHRs and telehealth platforms

Compensation/Benefits:

• Salary: $140,000 – $155,000 annually
