Job Requirements / Qualifications

  • Master’s degree in Social Work (MSW) required
  • LMSW required; LCSW preferred — Licensed Clinical Social Worker preferred for acute care social work
  • Minimum 2 years of healthcare or hospital social work experience
  • Experience with acute care case management, discharge planning, and behavioral health interventions highly desirable
  • Strong assessment, counseling, crisis intervention, and documentation skills
  • Familiarity with EMR systems (Meditech experience a plus)
  • Ability to work collaboratively in a multidisciplinary team including RN case managers and physicians
  • References, skills checklist, and license verification required

Key Responsibilities

  • Perform psychosocial assessments and evidence-based interventions for patients and families to address barriers to recovery
  • Identify high-risk patients and develop individualized care and discharge plans
  • Provide counseling and support to help patients and families adjust to illness, hospitalization, and care transitions
  • Advocate for patient needs and connect patients with community resources, home care, and entitlement programs
  • Complete timely, accurate documentation including social work notes and discharge planning assessments
  • Lead Ascend PASRR assessments for nursing home admissions and coordinate skilled nursing referrals
  • Coordinate referrals for home health, durable medical equipment, outpatient providers, and community services
  • Liaise with local, state, and federal agencies as needed and represent social work at hospital/community events
  • Collaborate closely with RN Case Managers and the interdisciplinary team for safe, efficient patient throughput
