Requirements / Qualifications

  • Active LPN (Licensed Practical Nurse) license in North Carolina or a Compact State (NC LPN license preferred)
  • Minimum 1 year of LPN clinical experience required; home health / homecare experience strongly preferred
  • Clinical skills in wound care, IV therapy, port care, and wound vac management
  • Experience with electronic documentation (HomeCare HomeBase / HCHB preferred)
  • Ability to rotate on-call weekends/evenings as needed
  • Strong communication, assessment, and documentation skills
  • Valid driver’s license and reliable transportation required for travel LPN duties in Charlotte and surrounding counties

Responsibilities — Home Health LPN

  • Provide patient-centered homecare nursing according to individualized care plans while promoting dignity and independence
  • Perform skilled nursing tasks: monitoring vitals, administering medications, enteral feedings, oxygen therapy, ostomy care, and wound treatments
  • Provide IV and port care per agency policy and report changes in patient condition promptly
  • Collaborate with interdisciplinary team members including PT, OT, and SLP to coordinate patient care
  • Maintain infection control and report communicable diseases or infection concerns
  • Complete accurate and timely electronic documentation of nursing actions, patient progress, and outcomes (HCHB experience a plus)
  • Manage supplies, medications, and equipment inventory for assigned caseload
  • Report incidents, complete incident reports, and follow agency escalation processes
  • Identify and report faulty medical equipment and coordinate timely repairs
