Qualifications / Job Requirements
- Active Licensed Practical Nurse (LPN) license in New Mexico or valid multistate compact LPN license
- Minimum 1 year of LPN clinical experience; long term care (LTC) experience preferred
- Ability to work 12-hour day shifts in a long-term care clinical setting
- Current CPR certification (BLS preferred)
- Strong communication, teamwork, and resident-centered care skills
- Experience with medication administration, documentation, and basic assessments
Responsibilities — LPN / LTC Nurse
- Provide compassionate, resident-focused nursing care in a long term care facility
- Administer medications and treatments per provider orders and facility protocols
- Monitor, assess, and document residents' health status, changes, and progress in the medical record (EMR)
- Assist residents with activities of daily living (ADLs) and promote independence and well-being
- Collaborate with the interdisciplinary healthcare team (RNs, CNAs, providers, therapists) to ensure high-quality care
- Follow facility policies, infection control procedures, and regulatory requirements
