• Duration: 14 weeks
• Shifts: Alternating schedule: One week 2x8 hour evening/night shifts, following week 3x8 hour evening/night shifts; variable schedule as needed; every other weekend and holiday required. Also, part-time option for 2x12 hour evening/night shifts per week, scheduled as needed by facility, every other weekend and holiday required.
• Weekly Estimated Pay: $1,170 – $1,230

Job Requirements / Qualifications

• Active NY LPN licensure (Licensed Practical Nurse)
• BLS certification through AHA or RQI
• Minimum 1 year of LPN experience in a long-term care facility (LTC / skilled nursing)
• Ability to begin full-day orientation on a Tuesday or Thursday

Key Responsibilities

• Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse (LPN resident care)
• Collect, document, and communicate clinical data about residents’ physical and psychosocial conditions to the RN and interdisciplinary team
• Contribute to development and revision of individualized care plans; ensure age-appropriate and culturally relevant interventions
• Collaborate with residents, families, and healthcare team members to promote a safe, therapeutic environment and achieve desired outcomes
• Maintain current nursing competency through continuing education and in-service programs (professional development for LPNs)
• Participate in mentoring and orientation of new staff as needed
• Demonstrate leadership, uphold safety standards, and adhere to infection control and bloodborne pathogen policies
