Job Requirements
- Active Licensed Practical Nurse (LPN) license in the United States
- Minimum 1 year of recent home health nursing experience with a Medicare-certified agency
- Proficiency with HCHB (Homecare Homebase) electronic medical record system
- Strong clinical assessment and care planning skills for long-term care and home health patients
- Ability to travel within Lewis County and adapt to changing patient locations — ideal for travel LPN roles
- Ability to work independently and collaboratively with an interdisciplinary healthcare team
Responsibilities
- Provide compassionate long-term care and skilled home health nursing services to patients in their homes and community settings
- Perform patient assessments, implement individualized care plans, and monitor clinical progress as an LPN / home health nurse
- Complete timely and accurate documentation using HCHB (Homecare Homebase) to meet Medicare and agency standards
- Coordinate care with clinicians, therapists, and case managers to ensure continuity of care for LTC and home health patients
- Support crossover assignments and flexible scheduling within the territory as needed
- Promote patient safety, medication administration, wound care, and other skilled nursing interventions appropriate to LPN scope
