Travel LPN – Inpatient Rehab | Madison, WI | 13-week contract • EPIC experience required • $2,500 completion bonus. - Position: Travel LPN / Licensed Practical Nurse (LPN) – Inpatient Rehabilitation
- Facility: Acute Inpatient Rehabilitation Hospital (Joint Commission & CARF-accredited)
- Contract Duration: 13 weeks (April 14, 2026 – July 14, 2026)
- Shift: 12-hour shifts (6:30am–7:00pm or 6:30pm–7:00am)
- Hours per Week: 36
- Schedule: Every other weekend and holiday rotations
- Estimated Weekly Salary: $1,300–$1,370/week
- Completion Bonus: $2,500 for full contract completion with satisfactory attendance
- Electronic Health Record: EPIC (minimum 13 weeks EPIC experience required)
- Job Type: Travel nursing job, LPN travel assignment
Qualifications / Job Requirements
- Active LPN/LVN license (must be eligible to practice in Wisconsin)
- BLS certification (American Heart Association preferred)
- Minimum 1 year of nursing experience in skilled nursing, inpatient rehab, or other 24/7 skilled care environment
- Inpatient rehab or med-surg experience preferred (rehab nurse experience a plus)
- Minimum 13 weeks of EPIC EHR experience required for charting and documentation
- Ability to assist with CNA duties as needed and support interdisciplinary care
- Strong teamwork, communication, and patient safety focus
Responsibilities
- Provide high-quality nursing care to adults requiring inpatient rehabilitation services, including patients referred from trauma centers
- Function as an integral part of the interdisciplinary rehab team to maximize patient recovery and quality of life
- Assist with mobility equipment (ceiling lifts, Sara Stedy, slide boards) and safe patient transfers
- Document patient care accurately and timely using EPIC EHR (rehab nursing documentation)
- Support CNAs, respond to call lights, and assist with activities of daily living as needed
- Participate in weekend and holiday rotations and adhere to facility attendance and scheduling policies
