Key Job Details – Vermont LPN Travel Contract
- Location: Middlebury, Vermont 05753 (Addison County)
- Healthcare Setting: Long Term Care & Memory Care Unit
- Contract Duration: 6 weeks (Start: 11/20/2025, End: 1/1/2026)
- Nursing Shift: 12-hour night shifts
- Hours per Week: 36
- Employment Type: Travel LPN Contract
- Estimated Weekly Salary: $2200 - $2360
- Floating: Required to Memory Care and Post-Acute Care (PAC) units
- Holiday Requirements: Must be available to work at least 2 out of 4 major holidays (Thanksgiving, Christmas, Christmas Eve, New Year's Day)
LPN Qualifications & Requirements
- Active Licensed Practical Nurse (LPN) license (Vermont or compact state)
- BLS certification (Basic Life Support)
- Minimum 1 year of clinical nursing experience (long term care or memory care experience preferred)
- Recent EPIC documentation system experience (within past 2 years)
- Ability to float to Memory Care and PAC units as needed
- No cat allergies (cat present on unit)
- Reliable personal or rental transportation (no public transportation available)
- Willingness to work scheduled holidays
- Dementia capable care certification preferred
LPN Responsibilities – Long Term Care Nursing
- Provide high-quality nursing care to residents aged 60+ in a long-term care and memory care setting
- Administer medications and treatments according to physician orders
- Accurately document patient care using EPIC EMR system
- Collaborate with interdisciplinary healthcare team members, including CNAs and ancillary staff
- Float to Memory Care and PAC units as required
- Participate in care planning and communicate with families
- Ensure compliance with facility policies, procedures, and healthcare regulations
- Maintain a safe, supportive, and compassionate environment for residents
