Job Requirements / Qualifications

• Active Licensed Practical Nurse (LPN) license in West Virginia (WV)
• Experience in long term care, skilled nursing, assisted living, or similar healthcare setting is preferred but not required
• Ability to work 12-hour day shifts and manage med pass, ADLs, and resident documentation
• Strong communication, teamwork, and resident-centered care skills
• BLS certification from the American Heart Association preferred
• Experience with electronic medical records (EMR) and charting is a plus

• Provide compassionate nursing care to residents in a long term care / skilled nursing facility
• Administer medications and treatments safely and accurately as prescribed
• Monitor, assess, and document patient status, condition changes, and care plans
• Collaborate with RNs, physicians, therapists, and interdisciplinary healthcare team members to ensure quality outcomes
• Assist residents with activities of daily living (ADLs) and promote overall well-being and dignity
• Support facility policies, infection control standards, and resident safety procedures
