Job Requirements

• Active LPN license in Pennsylvania (required)
• Minimum 1 year of experience in long-term care, skilled nursing, or related nursing setting
• Availability for evening shifts and every other weekend
• Strong communication, teamwork, and resident-focused care skills
• Commitment to compassionate, patient-centered care and resident safety

Responsibilities

• Provide high-quality nursing care to residents in a long-term care facility (LTC nurse duties)
• Administer medications and treatments as prescribed, including accurate medication administration and documentation
• Monitor and document resident progress, vital signs, and changes in condition
• Collaborate with interdisciplinary teams (nursing, therapy, social work) to ensure optimal resident outcomes
• Support residents with activities of daily living (ADLs) and promote resident dignity and well-being
• Maintain a safe, clean, and therapeutic environment for residents and staff
