Job Requirements / Qualifications

  • Active LPN license in Vermont or eligibility via multistate compact (Vermont LPN license preferred)
  • Experience with medication administration, resident assessments, documentation (EMR/charting), and ADL support
  • Ability to work night shifts and every weekend as scheduled (Night Shift LPN, LTC nurse)
  • Strong communication, teamwork, and resident-centered care skills
  • Reliable transportation required

Responsibilities

  • Provide compassionate, high-quality nursing care to residents in a long-term care/skilled nursing facility
  • Administer medications and treatments as prescribed and document accurately in the EMR
  • Perform routine assessments, monitor residents’ health status, and report changes to the interdisciplinary team
  • Assist residents with activities of daily living (ADLs) and promote dignity and independence
  • Collaborate with nursing staff, nursing assistants, physicians, therapists, and other healthcare professionals for comprehensive care planning
  • Maintain accurate clinical records, follow facility policies, and adhere to infection control and safety protocols
  • Support a positive resident experience and contribute to quality improvement initiatives
