Qualifications / Job Requirements

• Current LPN/LPN license in the applicable state (Licensed Practical Nurse)
• LTC, SNF, or skilled nursing experience preferred
• Ability to work consistent 12-hour night shifts (36 hrs/week) for the contract duration
• Clinical skills: medication administration, vital signs, focused assessments, documentation, wound care basics, and safety-focused resident care
• Professional skills: patient-centered care, strong communication, teamwork, dependability, and accurate charting

Key Responsibilities

• Provide direct nursing care to long-term care residents during night shifts, promoting dignity, comfort, and safety
• Administer medications and treatments per provider orders and facility policy; monitor residents for side effects and clinical changes
• Perform focused assessments and obtain vital signs; promptly report and escalate changes to the RN/charge nurse and provider as needed
• Document all resident care, medication administration, and resident responses accurately and on time per facility processes
• Collaborate with RNs, CNAs, and the interdisciplinary care team to support individualized care plans and continuity of care
• Follow infection prevention and control protocols and help maintain a clean, safe environment for residents
