Job Requirements
- Active, unrestricted LPN license (New Mexico or compact state)
- Minimum 1 year LPN experience, preferably in long term care, skilled nursing, or geriatric nursing
- Current BLS certification (American Heart Association preferred)
- Experience with medication administration, wound care, and clinical documentation (EMR experience a plus)
- Strong communication, teamwork, and resident-centered care skills
- Ability to adapt to a fast-paced LTC environment and prioritize patient safety and compliance
Responsibilities
- Deliver high-quality nursing care to residents in a long term care / skilled nursing facility
- Administer medications and treatments as prescribed; accurately document medication administration and clinical care
- Monitor, assess, and document residents’ health status, changes, and clinical progress
- Collaborate with interdisciplinary team members to develop and implement individualized care plans
- Provide resident and family education on care plans, medications, and chronic condition management
- Maintain a safe, sanitary, and compliant environment consistent with facility policies and regulatory standards
