Job Details

• Weekly Estimated Pay: $1,230 – $1,300 (competitive pay for LPN travel roles)
• Location: Uniontown, PA 15401 — LPN job in southwestern Pennsylvania
• Setting: Long-Term Care (LTC) / Skilled Nursing Facility
• Employment Type: Travel Contract (LPN travel nurse)
• Start Date: April 19, 2026
• End Date: June 14, 2026
• Duration: 8 weeks
• Shift: 8-hour evenings (3:00 PM – 11:30 PM); evening shift LPN
• Guaranteed Hours: 32 per week
• Hours Per Week: 40
• Weekend Rotation: Every other weekend required
• Set Rotation: Monday, Tuesday, Thursday, Saturday OR Sunday, Tuesday, Thursday, Friday

Qualifications / Job Requirements

• Active LPN license in Pennsylvania (required for LPN Uniontown PA positions)
• Minimum 1 year of LPN experience; long-term care (LTC) experience preferred
• Ability to work evening shifts and rotate weekends
• Strong communication, teamwork, and resident-focused care skills
• Current CPR certification (BLS preferred)

Responsibilities

• Provide compassionate, resident-centered nursing care in a long-term care/skilled nursing setting
• Medication administration and accurate documentation of medications and treatments
• Monitor, assess, and document resident health status and progress
• Collaborate with interdisciplinary team members — RNs, CNAs, social workers, therapists — to ensure quality care
• Assist residents with activities of daily living (ADLs) and support overall well-being
• Participate in care planning, discharge planning, and effective communication with families and caregivers
