Job Requirements

• Active LPN license
• Ability to work overnight shifts and adapt to the needs of LTC residents — experience as an LTC nurse or long-term care nurse preferred
• Strong communication, clinical documentation, and resident assessment skills
• Commitment to providing compassionate, patient-centered care to older adults

Responsibilities

• Provide direct nursing care to long-term care residents following facility policies and nursing standards
• Monitor, assess, and document residents' health status and report changes to the clinical team
• Administer medications and treatments safely and accurately as prescribed
• Assist residents with activities of daily living and promote resident comfort, dignity, and independence
• Collaborate with interdisciplinary team members, including RNs, CNAs, social workers, and therapists, to ensure continuity of care
• Maintain a safe, respectful environment for residents, visitors, and staff in the LTC facility
