Interim Case Management Manager Job Details

- Schedule: Monday through Friday, 8:00 a.m. to 4:30 p.m.

- Shift Duration: 8-hour day shifts
- Hours: Approximately 40 hours per week
- On-Call: Rotating Friday-to-Friday leadership coverage
- Weekly Estimated Pay: $3,134–$3,483
- Management Scope: Oversight of RN discharge planners and social workers; no individual patient caseload
RN Case Management Manager Job Requirements
- Active Registered Nurse (RN) license with eligibility to practice in New Jersey
- Bachelor of Science in Nursing (BSN) required
- At least 1 year of acute care case management leadership or management experience
- Experience supervising or leading RN case managers, discharge planners, social workers, or similar interdisciplinary healthcare teams
- Strong knowledge of discharge planning, utilization management, patient throughput, and interdisciplinary care coordination
- Ability to manage staffing assignments, coverage gaps, escalations, and operational priorities
- Epic electronic medical record experience strongly preferred
- Excellent communication, leadership, organization, and problem-solving skills
- Ability to begin the assignment promptly and work effectively in an on-site hospital leadership role
Acute Care Case Management Responsibilities
- Oversee daily operations for nursing discharge planners and social workers
- Manage staffing assignments, schedules, coverage needs, per-diem utilization, and payroll-related oversight
- Participate in daily multidisciplinary discharge rounds and promote timely patient transitions
- Identify, coordinate, and escalate barriers affecting discharge planning and patient throughput
- Serve as a leadership resource for patient and family concerns
- Coordinate with nursing leadership, physicians, ancillary departments, and hospital administration
- Support electronic medical record workflows, post-acute referrals, and authorization processes
- Complete daily case management reliability scorecards, staffing reports, and operational updates
- Provide leadership support for complex social needs and financial assistance processes when needed
- Coach and support team members while maintaining a collaborative, accountable department culture
- Provide rotating on-call leadership coverage for urgent operational and discharge-related matters
