Qualifications / Job Requirements
- Current Registered Nurse (RN) license in Michigan
- Minimum 2 years of clinical experience as a Home Health RN or hospice RN
- OASIS and HCHB (Homecare Homebase) experience required
- Current CPR certification (AHA preferred)
- Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation for travel nursing visits
Key Responsibilities
- Deliver compassionate home health and hospice nursing care to patients in Menominee and surrounding Michigan communities
- Perform patient assessments, develop individualized care plans, and document using OASIS and HCHB systems
- Coordinate care and communicate with interdisciplinary teams, physicians, therapists, and family caregivers to optimize patient outcomes
- Provide patient and family education on disease management, medication administration, wound care, and self-care techniques
- Maintain accurate clinical records and ensure compliance with Medicare, state, and agency regulatory requirements
