Hybrid Nurse Practitioner - Field Based
In-Home Chronic Kidney Care
San Antonio, TX
We are seeking a Nurse Practitioner or Physician Assistant to provide in-home care for patients with Chronic Kidney Disease (CKD) through End-Stage Renal Disease (ESRD) in a progressive, outcomes-driven model that blends home visits with telehealth support.
This is high-impact medicine — without weekends, call, or burnout scheduling.

What You’ll Do

Conduct comprehensive initial evaluations and follow-ups

Lead hospital-to-home transitions to reduce readmissions

Perform thorough head-to-toe assessments and medication management
-
Educate patients on CKD/ESRD disease progression and treatment plans
-
Collaborate closely with an RN, Social Worker, and supervising physician

Help prevent avoidable hospitalizations through proactive intervention
Visit Structure:

90 minutes for new patients
-
30–60 minutes for follow-ups

Ideal Candidate
-
Active, unrestricted NP or PA license
-
2+ years of direct patient care experience

Background in renal, chronic disease, internal medicine, home health, or complex care preferred

Comfortable working independently in a home-based setting
-
Passionate about improving care for medically complex populations
