RN Case Management Requirements
- Minimum of 3 years of RN case management experience required
- Recent hospital case management experience required
- EPIC experience required
- Graduate of an accredited or state board-approved nursing program
- Active Registered Nurse (RN) license required
- Flu vaccine required
Case Management RN Responsibilities
- Assist inpatients, outpatients, and their families in managing the plan of care across the continuum
- Coordinate care to support safe, efficient, and patient-centered outcomes
- Collaborate with physicians, nurses, social workers, and interdisciplinary teams to assess needs and facilitate appropriate services
- Support discharge planning and transitions of care
- Maintain accurate and timely documentation within EPIC and the electronic medical record (EMR)
- Promote quality care delivery while advocating for patients and families
