Job Requirements
- At least 1 year of recent experience in case management, care coordination, discharge planning, or social services
- Active Registered Nurse (RN) license or Licensed Social Worker (LSW) credential
- BLS certification required
- Experience in an acute care hospital or inpatient healthcare setting preferred
- Familiarity with Epic EMR preferred
- Strong communication, critical thinking, and interdisciplinary collaboration skills
- Ability to manage a patient caseload in a fast-paced hospital environment
Responsibilities
- Coordinate patient care plans and support transitions of care across the continuum
- Collaborate with physicians, nurses, social workers, and interdisciplinary care teams to promote quality outcomes
- Assist with discharge planning, resource coordination, and patient and family education
- Assess patient needs and connect individuals with appropriate community, post-acute, and follow-up services
- Document case management and care coordination activities accurately in Epic
- Support efficient utilization of services while maintaining a patient-centered approach
- Advocate for patients and families to ensure safe, timely, and appropriate care planning
