• Coverage Area: Wayne County, with possible support in nearby counties as needed
• Productivity Expectation: 30 points
• Travel Support: Mileage reimbursement available between visits

Job Requirements

• Active Physical Therapist license in the state of Kentucky
• Doctoral degree required
• Experience with HCHB EMR strongly preferred
• Ability to manage a home health caseload and work independently in the field
• Strong communication, documentation, and patient education skills
• Reliable transportation for patient visits throughout the service area

Responsibilities

• Evaluate patients to assess physical limitations, strength, balance, mobility, pain, and functional abilities
• Develop individualized treatment plans based on physician orders and patient needs
• Provide therapeutic interventions such as exercise programs, gait training, balance work, strengthening, endurance activities, and pain management techniques
• Teach patients and caregivers safe movement techniques, home exercise programs, and strategies to improve daily functioning
• Assess home safety and recommend adaptive equipment or environmental modifications to reduce fall risk
• Monitor patient progress and adjust treatment plans to support goal achievement
• Document care and outcomes according to agency, state, Medicare, and professional standards
• Coordinate care with physicians, nurses, occupational therapists, speech therapists, social workers, and other members of the care team
• Educate patients on disease management and recovery following surgery, injury, illness, or hospitalization
• Maintain compliance with regulatory requirements, quality standards, and agency policies
