- Coverage Area: Jackson and Swain Counties, NC
- Dress Code: Business casual or dark blue/gray scrubs
Job Requirements
- Current North Carolina Physical Therapist license
- At least 1 year of Physical Therapist experience preferred or required
- Previous home health Physical Therapy experience preferred
- OASIS experience required
- HCHB experience preferred and will help candidates stand out
- Ability to work independently and manage a home health caseload
- Strong communication, documentation, and patient education skills
Responsibilities
- Complete home health Physical Therapy SOC visits, evaluations, reassessments, discharges, and routine visits
- Provide skilled therapy services in patient homes throughout Jackson and Swain Counties
- Develop and implement individualized care plans to support patient recovery, mobility, and functional goals
- Accurately document visits and clinical findings in a timely manner
- Collaborate with the interdisciplinary care team to support safe, effective patient outcomes
- Maintain productivity expectations while delivering high-quality, patient-centered care
- Travel between visits as needed with mileage reimbursement provided
