- Coverage Area: Home visits throughout nearby Northern Virginia communities
Job Requirements
- Active Virginia Physical Therapist license required
- Minimum of 1 year of Physical Therapy experience required
- Previous Home Health Physical Therapy experience required
- Experience with HomeCare HomeBase (HCHB) documentation required
- Experience with PointCare documentation required
- Strong communication, organization, and time-management skills
Responsibilities
- Provide skilled physical therapy services to patients in the home health setting
- Evaluate patient needs and develop individualized treatment plans
- Document care accurately and efficiently using HCHB and PointCare
- Track patient progress and adjust treatment plans as needed
- Educate patients and caregivers on therapy goals, exercises, and safety
- Travel between scheduled visits throughout the assigned service area
- Maintain productivity standards while delivering high-quality patient care
