  • Location: On-site, covering Charlottesville and Staunton, VA

  • Contract Length: 8 weeks
  • Estimated Weekly Pay: $1,936 - $2,152
  • Schedule: Monday - Friday, flexible hours (8:30am - 5:00pm)
  • Shifts per Week: 5
  • Shift Duration: 8-hour days
  • Hours per Week: 40
  • On-Call: 1 week per month (easy rotation)
  • Mileage Reimbursement: $0.45 per mile

RN Case Manager Qualifications

  • Active, unrestricted Multi-State Registered Nurse (RN) license
  • Associate Degree in Nursing or related field required
  • Minimum 1 year of nursing experience (home health, case management, or clinical support preferred)
  • Case Management certification
  • Experience with OASIS and Homecare Homebase (HCHB) required
  • Current TB test
  • Wound/Competency test
  • CPR/BLS certification
  • Strong clinical assessment and decision-making skills

RN Case Management Responsibilities

  • Conduct comprehensive assessments of high-risk patients in their homes
  • Collaborate with primary care providers to develop and implement individualized care plans
  • Monitor and reassess care plans for cost-effective, appropriate levels of care
  • Coordinate non-medical support services (housing, transportation) to support patient compliance
  • Manage care transitions and coordinate with physicians, social workers, and discharge planners
  • Engage specialty healthcare resources for optimal patient outcomes
  • Maintain accurate records of clinical, functional, and fiscal outcomes
  • Promote health education and illness prevention strategies
  • Act promptly to prevent adverse patient events and minimize risks
  • Advocate for patient privacy and confidentiality
