- Shift: 8-hour days, Monday–Friday (On-call rotation: 1 weekend per month)
- Location: On-site in Mount Sterling, KY (40353) — serving Nicholas, Montgomery, and Menifee Counties
- Mileage Reimbursement: $0.43 per mile
Job Requirements
- Active Kentucky RN license required (Compact license accepted)
- Minimum 1 year of nursing experience; home health nursing or case management experience preferred
- Experience with HCHB (Homecare Homebase) strongly preferred
- Clinical skills preferred: PICC line care, wound care, negative pressure therapy (NPWT)
- Ability to meet benchmark requirements (30 points)
- Strong documentation, patient education, and care coordination skills
Responsibilities
- Provide comprehensive case management and skilled nursing care to home health patients
- Conduct on-site patient visits across Nicholas, Montgomery, and Menifee Counties (home visits required)
- Perform wound care, PICC line management, and negative pressure wound therapy as indicated
- Document clinical care accurately in HCHB electronic health record (Homecare Homebase)
- Participate in on-call rotation (1 weekend per month) and coordinate timely care interventions
- Collaborate with interdisciplinary teams (physicians, therapists, case managers) to ensure optimal patient outcomes
- Educate patients and caregivers on home care plans, medications, and self-care techniques
