RN Case Manager – Hospice Job Details
- Job Title: Travel Registered Nurse (RN) Case Manager – Hospice
- Location: York & Lancaster Counties, Pennsylvania (On-Site) – Pennsylvania nursing jobs
- Start Date: November 24, 2025
- Contract Length: 8-week travel assignment
- Schedule: 5 shifts/week (Monday – Friday), 8-hour days (8:00am – 4:30pm)
- Hours: 40 hours/week
- Estimated Weekly Pay: $1,884 – $2,094
- Openings: 2 RN positions
- Employment Type: Travel Contract (W2)
- No weekends or on-call required
Registered Nurse Qualifications
- Active, unrestricted Registered Nurse (RN) license in Pennsylvania
- Associate's or Bachelor's Degree in Nursing or related field
- Minimum 1 year of RN experience (preferred: hospice nursing jobs experience)
- Hospice and HCHB (Homecare Homebase) experience required
- Valid Driver's License and current car insurance
- Current BLS CPR certification
- Case Management certification preferred
- Strong clinical assessment, decision-making, and communication skills
RN Case Manager Responsibilities
- Conduct comprehensive assessments of high-risk hospice patients in home and community settings
- Complete admission, recertification, routine, PRN, and death visits (5–7 visits per day)
- Develop and implement individualized patient care plans in collaboration with primary care providers
- Reassess and ensure cost-effective, appropriate levels of hospice care
- Arrange non-medical support services (housing, transportation) to promote treatment compliance
- Coordinate care transitions and collaborate with physicians, social workers, and discharge planners
- Engage specialty healthcare resources to achieve optimal patient outcomes
- Maintain accurate records of clinical, functional, and fiscal outcomes
- Promote health and prevent illness through patient education and advocacy
- Protect patient privacy and confidentiality as a dedicated advocate
- Respond promptly to adverse patient events to minimize negative outcomes
