Registered Nurse – Case Management Hospice Travel Job in Hyannis, Massachusetts
Seeking an experienced Registered Nurse (RN) for a 13-week travel nursing assignment in case management with a hospice-focused patient population in Hyannis, MA. This on-site hospice RN role offers the opportunity to provide skilled nursing care, supportive care, and pain and symptom management while helping patients and families navigate care with compassion and professionalism in beautiful Cape Cod healthcare settings.

- Weekly Estimated Pay: $2,168 - $2,409
- Location: Hyannis, Massachusetts
- Healthcare Setting: On-site hospice and home-based patient care
- Start Date: 8/3/2026
- Assignment Length: 13-week travel assignment
- Schedule: Monday-Friday, 8:00 AM-4:00 PM, with occasional evenings and weekends
- Shifts per Week: 5
- Shift Duration: 8 Hour Days
- Hours per Week: 40
Job Requirements
- Current Registered Nurse license in Massachusetts
- At least 1 year of nursing experience required, or 6 months of nursing experience with a BSN
- Hospice nursing or home care experience preferred
- IV certification preferred
- Strong computer skills and electronic documentation proficiency
- BSN preferred
- Successful completion of pre-hire medication testing
- Excellent communication, critical thinking, and time management skills
Responsibilities
- Provide skilled nursing care, supportive care, and pain and symptom management for hospice patients in homes and facilities
- Participate in interdisciplinary patient case conferences as needed
- Assess patients, identify changes in condition, and respond appropriately
- Communicate significant patient changes to physicians and other care providers
- Complete admissions, documentation, and required paperwork in a timely manner
- Maintain clear, concise narrative documentation reflecting skilled care, teaching, and patient or caregiver response
- Obtain and clarify valid physician orders for care delivered
- Explain hospice philosophy, scope, and services to patients, families, and facility staff
- Implement the plan of care under physician direction in compliance with federal and state regulations
- Supervise hospice aides in accordance with regulatory requirements
- Support team development by assisting with orientation and mentoring of new staff when requested
