- Schedule: Monday to Friday, 8:00 AM to 4:30 PM
- Shift Duration: 8 Hour Days
- Shifts Per Week: 5
- Hours Per Week: 40
- Contract Length: 13 weeks
- Patient Population: Adult
- Patient Ratio: 1:15
- Bed Count: 200
- Charting System: EPIC
- Floating: Required
- Weekend Commitment: Up to every other weekend
Job Requirements
- 10+ years of Registered Nurse experience required
- Recent case management experience in an inpatient healthcare setting
- Strong background in utilization management and discharge planning
- Active RN license required
- AHA BLS certification required
- Certified Case Manager (CCM) preferred
- Ability to work effectively with adult patient populations and interdisciplinary care teams
- Experience with EPIC preferred
Responsibilities
- Coordinate care for adult inpatient medical-surgical patients across the continuum of care
- Perform comprehensive case management assessments and develop appropriate care plans
- Support utilization review activities to promote appropriate level of care and resource use
- Lead discharge planning efforts to ensure safe, timely, and effective transitions of care
- Collaborate with physicians, nursing staff, social services, and ancillary departments
- Identify barriers to discharge and help facilitate solutions for patients and families
- Document accurately and efficiently in EPIC
- Float as needed based on departmental needs
