  • Schedule: Monday to Friday, 8:00 AM to 4:30 PM

Job Requirements

  • 10+ years of Registered Nurse experience required
  • Recent case management experience in an inpatient healthcare setting
  • Strong background in utilization management and discharge planning
  • Active RN license required
  • AHA BLS certification required
  • Certified Case Manager (CCM) preferred
  • Ability to work effectively with adult patient populations and interdisciplinary care teams
  • Experience with EPIC preferred

Responsibilities

  • Coordinate care for adult inpatient medical-surgical patients across the continuum of care
  • Perform comprehensive case management assessments and develop appropriate care plans
  • Support utilization review activities to promote appropriate level of care and resource use
  • Lead discharge planning efforts to ensure safe, timely, and effective transitions of care
  • Collaborate with physicians, nursing staff, social services, and ancillary departments
  • Identify barriers to discharge and help facilitate solutions for patients and families
  • Document accurately and efficiently in EPIC
  • Float as needed based on departmental needs
