- Employment Type: Travel Contract (Travel Nursing, Travel RN)
- Duration: 13 weeks
- Hours Per Week: 40
- Shift: 4x10-Hour Days (08:00 - 18:30)
- Start Date: May 18, 2026
- On-site: Yes — ability to float to other facilities within the leading academic medical center required
RN Job Requirements (California)
- Active California Registered Nurse (RN) license
- AHA BLS Certification required
- Minimum 2 years of recent acute hospital experience in case management, discharge planning, or clinical nursing
- Experience with discharge planning, utilization review, or care coordination in an acute care setting
- Strong written and oral communication, diplomacy, organizational, and analytic skills
- Self-directed, assertive, and creative problem-solving skills for systems planning and patient care management in a high-volume environment
- Ability to float to other facilities within the leading academic medical center as needed
- Preferred: Master’s Degree in Nursing, Healthcare Administration, or related field
Responsibilities – Clinical Case Management
- Coordinate and manage patient care and discharge planning for acute hospital patients to ensure safe transitions of care
- Provide clinical case management, care coordination, and utilization review to improve patient outcomes
- Collaborate with interdisciplinary teams including physicians, social workers, therapists, and community providers
- Document care plans, discharge summaries, and care coordination activities in the electronic health record
- Demonstrate resourcefulness and creativity in problem-solving, systems planning, and patient advocacy
- Manage complex cases in a fast-paced, high-volume hospital environment
- Float to other facilities within the leading academic medical center as required to support clinical operations and patient care needs
