Job Details
- Position: Home Health Registered Nurse (RN) / Homecare RN / Home Health Nurse
- Weekly Estimated Pay: $2,190 – $2,550
- Location: Mission Viejo, CA (serving Newport Beach, Costa Mesa, Tustin, Irvine, Lake Forest) — Orange County nursing jobs
- Schedule: Thursday–Monday or Friday–Tuesday, 8:00 AM – 5:00 PM
- Shifts per Week: 5 (8-hour days)
- Guaranteed Hours: 40 per week
- Contract Length: 13 weeks (travel contract / temporary home health assignment)
- On Call: None
- EMR: HCHB (Homecare Homebase) required
- Productivity: 27.5 – 30 points per week
Job Requirements / Qualifications
- Active Registered Nurse (RN) license in California
- Minimum 1 year recent home health nursing experience with a Medicare-certified home health agency
- Proficiency with HCHB (Homecare Homebase) EMR system
- Strong clinical assessment, documentation, and care plan development skills
- Experience with medication administration, wound care, skilled nursing procedures, and patient education
- Valid driver's license and reliable transportation for home visits across Orange County
Key Responsibilities (Home Health Nurse)
- Provide skilled nursing care to patients in their homes across the assigned Orange County territory
- Conduct comprehensive assessments and develop individualized home health care plans
- Administer medications, perform wound care, and deliver other nursing interventions as prescribed
- Educate patients and families on disease processes, medications, and self-care techniques to support recovery and prevent readmissions
- Collaborate with interdisciplinary team members, including case managers and therapists, to ensure coordinated post-acute care
- Maintain accurate, timely documentation in HCHB EMR to meet Medicare-certified agency and regulatory standards
- Meet productivity standards of 27.5 – 30 points per week while delivering high-quality, patient-centered care
