Job Details

  • Position: Home Health Registered Nurse (RN) / Homecare RN / Home Health Nurse
  • Weekly Estimated Pay: $2,190 – $2,550
  • Location: Mission Viejo, CA (serving Newport Beach, Costa Mesa, Tustin, Irvine, Lake Forest) — Orange County nursing jobs
  • Schedule: Thursday–Monday or Friday–Tuesday, 8:00 AM – 5:00 PM
  • Shifts per Week: 5 (8-hour days)
  • Guaranteed Hours: 40 per week
  • Contract Length: 13 weeks (travel contract / temporary home health assignment)
  • On Call: None
  • EMR: HCHB (Homecare Homebase) required
  • Productivity: 27.5 – 30 points per week

Job Requirements / Qualifications

  • Active Registered Nurse (RN) license in California
  • Minimum 1 year recent home health nursing experience with a Medicare-certified home health agency
  • Proficiency with HCHB (Homecare Homebase) EMR system
  • Strong clinical assessment, documentation, and care plan development skills
  • Experience with medication administration, wound care, skilled nursing procedures, and patient education
  • Valid driver's license and reliable transportation for home visits across Orange County

Key Responsibilities (Home Health Nurse)

  • Provide skilled nursing care to patients in their homes across the assigned Orange County territory
  • Conduct comprehensive assessments and develop individualized home health care plans
  • Administer medications, perform wound care, and deliver other nursing interventions as prescribed
  • Educate patients and families on disease processes, medications, and self-care techniques to support recovery and prevent readmissions
  • Collaborate with interdisciplinary team members, including case managers and therapists, to ensure coordinated post-acute care
  • Maintain accurate, timely documentation in HCHB EMR to meet Medicare-certified agency and regulatory standards
  • Meet productivity standards of 27.5 – 30 points per week while delivering high-quality, patient-centered care
